What information do I need to bring with me to my office visit?
You will need to bring your insurance cards, a list of all medications you currently take, and any medical records, test results, or films relating to your visit to us.
In addition, please come prepared with a Primary Care Physician Referral or Copay, as required by your insurance company.
If this is a follow up visit, please make us aware of any changes in medications or insurance.
How do I know if my insurance requires a referral or authorization?
If your insurance is an HMO, a referral by your Primary Care Physician is required. If you were required to choose a Primary Care Physician when you signed up for insurance benefits, you probably need a referral.
Most tests are covered by insurance companies as long as your physician believes it is medically necessary.
If you have questions about coverage specifics, please call your insurance company.
How do I get copies of my medical records?
Send a signed release form to our office which includes your name, your doctor's name, and the address to send the records and we will forward your records.
How do I get my prescription refilled?
When you need a prescription refilled, please contact the pharmacy, which will contact our office to obtain approval. Or you may contact our office directly and request a refill on your medication. Please have your pharmacy telephone number available for our office.
