Inpatient Hospital Appointments - University of Miami Hospital

TO SCHEDULE YOUR HOSPITAL ADMISSION OR TRANSFER FROM ANOTHER HOSPITAL

Please call scheduling at 305-325-4516 to arrange your hospital admission date and time.

PREPARING FOR YOUR HOSPITAL ADMISSION

Please obtain copies of all medical records, blood test results, and x-ray studies from your personal physician and hospital. We suggest that you keep copies of your important medical records. We also request that you do not mail important medical information because it may not arrive in time for your admission or transfer. If you have undergone a liver biopsy, please contact your hospital's pathology department where the biopsy was performed and request the actual slides to be provided to you so that you may bring them with you to your inpatient admission. It is also helpful to bring a list of all current medications including herbal and over-the-counter medications.

YOUR HOSPITAL ADMISSION

Please go to the admitting office located in the 6th floor lobby area of the University of Miami Hospital to be registered for the admission. Hand carry with you your insurance card and any pertinent medical records.

DRIVING DIRECTIONS

From the North, South, and East

• Take I -95 to Exit 6 State Road 836 West (Dolphin Expressway)
• Exit at NW 12th Avenue
• Turn right (north) on NW 12th Avenue and to NW 154th Street
• Turn left on 15th Street to hospital ramp on left
• Entrance to University of Miami Hospital on the left side at 15th Street

From the West

• Take State Route 836 East (Dolphin Expressway) to the 17th Avenue Exit
• Follow the hospital signs to NW 12 Avenue
• Turn left (north) on NW 12 Avenue
• Turn left on 15th Street to hospital ramp on left
• Entrance to University of Miami Hospital on the left side at 15th Street
